Initial Consultation Deposit

I authorize Hudson Valley Veterinary Dermatology, PLLC to charge my credit/debit card $150 upon scheduling an initial consultation and agree to honor this in accordance with the issuing bank cardholder agreement and the terms of this transaction. I understand that this fee will be applied toward Charges incurred at the first consultation only, and is refundable only upon a minimum of 24 hours notice of cancellation.
